What We Consider

When assessing referrals, we consider:

This ensures that placements are sustainable and that individuals have the best opportunity to succeed.

After Acceptance

If a referral is accepted, we will coordinate with the referring agency to arrange move-in arrangements and provide details of the allocated Housing and Support Worker. A personalised needs assessment, support plan and risk assessment will be developed shortly after move-in to ensure structured, goal-focused support from the outset.

Partnership Working

We value strong partnership working and maintain open communication with referring agencies throughout a resident’s placement. Regular updates can be provided where appropriate, in line with data protection and confidentiality requirements.
